The Health Care Centre is a well-established, fully accredited Medical General Practice.
We have been caring for the Upper Lachlan Shire community since 2008 and believe each patient is important. We will take the time to get to know you so as to provide the best possible comprehensive and professional medical care.
All our doctors are vocationally registered and we are a registered training practice for GP Registrars to complete their training.

NEW PATIENTS
To make a booking, New Patients are to call 02 4843 2500 to book an appointment. Where possible, please complete the details form by selecting Patient Details form link and submit it to our Reception Team on arrival.
Payment is required at the time of consultation and we accept EFTPOS, credit card, cash and cheques.
Concession fees are available for pensioners and children under 16 years of age.
Medicare rebates can be claimed at the time of consultation.

PATIENT PARKING AND ACCESS
The Health Care Centre is located at 17 Kialla Road, Crookwell. Main public access is at front of the building where we have two disabled car park spaces and ramp access.
If you are experiencing difficulty with mobility, we can offer ramp access around the corner on McDonald St. The area is level and ramped for easy access.
If you require assistance to or from your car, please advise staff when booking an appointment.
